It's not a mutual decision that both parties need to resolve -- it's old underwear. A nice bra with the tags on in her size? I mean, it depends on the financial situation, but I wouldn't turn my nose up, regardless of who bought the thing. And open communication which acknowledges the awkwardness severely limits the possibility that she's going to find out about it later and think he's keeping secrets, so yeah, it is a communication issue on that level. The problem is that the less open he is, the more likely she is to think there's something to find out. He throws the bra in the trash--she sees it and thinks what? So he throws it in an outside dumpster or donates it--but what if someone sees him and tells her?
He's completely innocent of wrongdoing, but the more secretive he is, the more guilty he looks. The whole reason he's here asking is he doesn't want her to get the wrong idea. Telling her up front and being honest about how weird the situation feels makes it way less likely she'll have any room to make erroneous assumptions. Because cheaters just don't turn up with a bra and say "hey this is awkward, but my ex must have left this here at some point and the first thing I did was loop you in.

If I am understanding you correctly, it's a friend of your girlfriend's family who passed away? If that is the case, it could just be that she is dealing with grief and feelings of loss and sadness, and that is why she is pulling away from you.
I don't think you need to go out and find a new girlfriend unless there are other issues in the relationship that you feel are problematic. It's possible that her feelings have changed and she isn't as interested in you as she once was, but I don't get the feeling that that is the case.
What I would advise is sitting down with her and letting her know that you are concerned about her and you want to talk about what's going on. Let her know that she can talk to you -- show her that you care about her and want to understand why she seems to be pulling away from you.
I think you can clear up this situation if you can show some concern, love and compassion -- all you need is to get her to open up to you and share her feelings. Then you can better decide as to how you want to proceed. But I think you need some more information first.
